multiple teeth/molar extractions - difficulty to swallow and major speech difficulty

Had 8 teeth/molars extracted in dental chair,  local anaesthetic xylotox e80-a (vasoconstrictor) 9 injections in gums under top lip and injections in pallette - during  2nd to last injection in gum under lip at back upper i felt pain like lightning bolt shoot through face i was given an extra shot after this and my face went numb to above eyebrows, ear and behind ear, battled to move eyes and tears streamed. what caused this pain and why did i go numb to this extent, i also want to know why i did not bleed when i normally bleed even after i extraction. denture could not be retained in mouth immediately, i gagged and when i spoke it was so slurred i could not be understood , i had great difficulty swallowing even liquid. it is 2 and a half yrs now and i still have loss of sensation in my face mouth and throat i must  chin tuck to swallow and speech is  so bad i cant be understood my tongue is thick with fasiculations and i now know that is the reason i cant swallow or talk as i cant form the words. whole procedure from start to end was done in 20 mins - i was officially authorised for in hospital under general anaesthetic but dentist went on to perform procedure in chair local anaesthetic even though it was his opinion teeth needed to be extracted under general anaesthetic. could this be the result of damage caused by embolum injection and would you pick that up on an mri. my mri was normal.  what quantity of local would have been minimum effective dose for this procedure it was done in one sitting,  teeth/ molars were from left side face 7 teeth across and then a bridge of two teeth cemented into  last molar right hand back just before wisdom tooth. what else could have gone wrong.     I was 100% normal when i walked in but suffered these complications with immediate effect after 20 mins in dental chair.
