I keep on getting pins and needles (sharply) across the top of my chest.  They are very sore accompanied by a degree of tightness and goes through to my upper back.

This sounds like inflammation of nerves called neuritis.  There are many superficial nerves in the skin that overlies the chest.  The herpes zoster virus is a common cause of this, commonly called "shingles".  If this is the cause and the onset recent, therapy may effectively provide relief.
