HELP

I have intervertebral disc space narrowing at L3-4 with osteophyte formation, and Diffuse annular bulging  at this level,L4-5 and L5-S1. Changes are degenerative. There is Mild osteophytic lipping throughout the lumbar spine, and Schmorl's node formation is seen within the anterior superior end plate of L4. Does this really require surgery? if so what kind? My symptoms is Back pain all the time, leg pain, and weakness, and back leg and feet numbness. It hurts but I want to do the right thing.
