Chronic Denervation-C5-shoulder pain

My neck/shoulder/back is really bothering me.  I had a EMG that revealed  loss of motor unit in the infra and supraspinatus.  In addition, there is significant 39% units in the lower cdervical paraspinous muscles on the left.  Also, there is chronic denervation in the supra and infraspinatus, and due to signifcant change in the paraspinous muscles (cervical), C5 radiculopathy should be considered (left).  WHAT DOES ALL THIS TERMINOLOGY MEAN?  Is this serious or will I need ongoing physical therapy or SURGERY. If surgery is an option, will it be worth it?   It's a give that I will need to continue my pain meds.  Thank you.
