Coverage Termed, Ins Still Paying??

I changed employers and coverage termed 06/06. We had dual insurance and my old ins is still paying. Not intentional, but had a hysterectomy 6 weeks ago and hosptial had old info, added new info instead of replacing, billed old insurance and they paid on my surgery. I contacted them in July 06 about them paying a claim from my drs office and they assured me it was just a mistake and they "fixed it while I was on phone' Thanked me for calling them as most people would not. Hospital called before surgery to get pre-auth and was told it was no longer in effect, yet they still billed??? Insurance company told them it was no longer in effect and they still paid? What is going on? Am I going to end up getting a bill from insurance company for amounts they have paid? What else can I do, multiple calls about this seem to do no good in getting it corrected? This should not be an issue as all drs only have my husbands insurance info but still... I don't want this to come back on me. Is that possible? Help!!!! Thanks!
