muscle twitching-HELP?

I am 35y/o WF with upper and lower extremity muscle along with neck spasms with prologed use.  This has been an issue for 10+ years but now I am getting fasiculations with minimal  overuse.  When overused.... my muscles fatigue and I cannot use till I rest them. I take NSAIDS and muscle relaxants to help with discomfort.  Any ideas on what  I may be dealing with?  Oh. I also appear to have some mild loss in muscle mass of my forearms and hands.  I also was recently diagnosed with thyroiditis.  Any suggestions of where to begin or what I may be dealing with?
