Suspected Penile Cancer

Dr. Rockoff,

I am a 25 year old neonatally circumsiced male from the midwest.. During my youth I masturbated with different materials. As a result, I was afflicted with REPEATED episodes of acute dermititis that would result in redness, irritation, urinating with a "sprinkler" effect, etc. etc.

Then, beginning in approximately 2002-3 I slowly began to develop a "dot" about the size of a pencil eraser on the shaft of my penis, as well as a reddish-purplish rash. The rash begins at the circumcision line and ends at the tip of the penis.

In addition, I have a wart-like growth at the tip of my penis right at the oppening that is slightly raised and varies in levels of irritation, increasing with masterbation and sexual acitivity. Most disturbingly I have spend these last few years suffering with CONSTANT dysuria that varies in intensity, as well as swelling of the penis from the circumsion line to the head which comes and goes, but appears at least once daily.

Finally, I have seen two M.D.'s and a dermatologist who have assured me that it is not cancer, but are unsure of what it is. I have had a urethral swab taken that I believe came back negative, and in 2005 i had a urinalysis which came back "no bacteria present."

I fear that I may be over exaggerating my internal symptoms due to my unparalleled anxiety in regard to this issue, but the physical symptoms are present without a doubt. I had been masterbating throughout this time but with far less frequency and intensity.
The last time I have seen a doctor in pertinence to this issue was 2005, Do you think it would be a result of trauma that I have not allowed to heal, the dark pigmented skin being simply dead rather than cancerous?

First of all, circumcised men don't get penile cancer.  Second, cancer doesn't look like flat pigmentation.  Third, freckle-like spots are often present on the penis and don't mean anything.  The urinary symptoms have nothing to do with the skin (except, perhaps, from anxiety about the skin, as you point out yourself.)  See another dermatologist if you need to for reassurance, but it's time I think for you to get past your anxiety, which is almost certainly baseless.  Get any psychological help you need to achieve this.
