negative dysphotopsia

I have negative dysphotopsia. Shadow in tempoal region of vision. Very disturbing. Four surgeries
this year for various problems incld. retinal detachment and YAG capsulotomy. Need suggestions.

My personal suggestion is to avoid further surgery if at all possible.  Replacing the IOL may not do anything for the problem unless the IOL is really out of position or has a very small optic (like 5mm).  You have had a yag so you should really leave the IOL.  I know one very well know surgeon who has advocated a piggyback IOL in certain dysphotopsia situations - but mostly anecdotal reports - no strong scientific evidence that it works.  Try every possible drop that could possibly help reduce the pupil size (pilocarpine? Alphagan?)  Try very, very hard to adapt and avoid more surgery.  Sometimes just knowing that the problem is not really fixable can help.  Now I can't really comment on your particular case, because I don't know the particular cause or treatment.  I'm just making generalized comments.
