emergency room billing

My daughter went to the emergency room and was treated for for what seem ed to be a spider bite. The doctor treated three places and each bite was charged as a separate entity. The bill was close to $3,00.00. Included in this bill was the doctor's bill which she later received a separate bill for. Is this an example of bill padding or a possible mistakeor is this the way it is suppose to be?

Any time you go to the Emergency room.. everything that is done.. every doctor that you see  or medicine/test given.. ANYTHING... is always seperate...
