weird bluriness part 2

by speedygarcia, Oct 27, 2007 01:16PM
Dear Eye Doctor,
Thanks for your quick response. I am posting this question again becuase I made a mistake and didn't know how to respons to the other question I had posted a couple of days back. My dad has seeked the best eye doctors in town, and has gotten second and third opinions. This is why I seeked this forum, for even more opinions. I also asked my dad if I had posted this comment correctly and he made some corrections. Sorry for that!!! Please find below the modified history:

My dad had a surgery done on his right eye at the end of July. His cataracts were pretty mature and the procedure the eye doctor used is phako-emulsification. Right after the operation, my dad could not see anything clearly, his vision was very blurry. The eye doctor prescribed him some drops:
PREDNISOLONE ACETATE lO mg. (every 6 hours) and CILODEX (CIPROFLOXACINO WITH DEXAMETAXONE) drops (every 6 hours). He used the drops for about 2 months and the doctor took these away gradually.

The doctors told my dad that he has a corneal edema in the DESCEMET membrane. After the time went by, his vision was getting better and better, little by little, until one day (about two weeks ago) he suffered from a great pain in his eye. His vision went back almost to the state it was right after the operation: very blurry!!! He had big pain and the eye itched a bit. Another doctor also saw that there was a RUPTURE OF THE POSTERIOR CAPSULE (cristaline capsule). When he went to the doctor who operated his eye, he thought there was some kind of infection and prescribed my dad eritromicine drops (0.5%) and Coxylan (MELOXICAM), both taken one drop 3 times a day. The eritromicine drops gave him a weird feeling in his eye, like if he had something in his eye, itchiness and it stung, so he only used these for five days. Currently, he is just using Systane (conjunctive lubricant).

My dad's eye is not hurting anymore. The operating eye doctor says that his eye is ok, he took his eye pressure and it was 12 and 13. The problem is that it is not clear why he was improving and now, after this great pain (two weeks back), his vision went back to being very blurry. He also said that he has the PSEUDOEXFOLIATIVE SYNDROME. There is also no clear reason why he sees very blurry in the morning and it gets a bit better in the afternoon (still blurry but not so bad as in the morning).

The last eye prescription he got in December 2006 was:
Right eye: _O.50 x 90     5 prisma, external base  l95º•
Left Eye: +2.OO_0.25 x 90º  6 prisma, external base 0º

Dip 62 mm / 59mm
Ad + 2.75       Flat top

His eyeglasses need prisms because he sees double (diplopia).

And the doctor implanted the following lens in his eye:
model SN6OAT power 21.OD
            Length(oT): 13.0mm
            Optic(o B): 6.0mm
            SN:147709.036

My questions are:
1. What is going on with his eye, is he still seeing blurry because of this corneal edema? How much longer do we have to wait for his vision to be back?
2. What caused this big eye pain?
3. Why did his vision get worse, why is he seeing even more blurry than before, when he was already getting better little by little?
4. Why can he see a bit better in the afternoon than at night?
5. Are there any complications because of the RUPTURE OF THE POSTERIOR CAPSULE?
6. When will his corneal edema go away?

Thank you very much in advance for your answers and advice and sorry for the confusion!!!!

I'm sorry but I have to repeat that there are so many loose ends in your father's case that I cannot even generalize about the cause or prognosis. I suggest again that you get a secon opinion perhaps with a Eye MD that specializes in cornea surgery.  We only comment when we feel we can be informative. In this case I cannot.
