Acoustic Neuroma

I have an acoustic neuroma. It's 12.1 mm wide x 10.1 AP x 7.8 cc mm in Size. There is also a 20 mm cystic component. I am confused if I can watch and wait. I have posted at the Acoustic Neuroma Foundation and the ANA Archives and other places, and have gotten so many contradictory answers it's amazing. My symptoms are hearing loss, pain and pressure in that ear, and lately dizziness, not spinning but lightheadedness. Why does there seem to be so much agreement about what to do for these things? Does the cyst mean surgery is really needed?

From what I understand, I have a Pilocytic Astrocytoma, mine had a cystic component also; however, watching and waiting wasn't an option. From reading about acoustic neuromas, if they are small enough, they can be watched; however, if they become large enough they can start causing damage to surrounding structures. They (acoustic neuromas) are slow-growing so most times initially drs. don't automatically rush you in for surgery, but my guess will be that some type of intervention will be needed eventually. Treatment of acoustic neuromas basically fall into three categories: conservative, radiotherapy, and surgery, which is the recommended definitive treatment. The probable reason why you have been getting so many contradictory advices on what to do stems from the fact that the doctors answering your query in the online forums cannot give you a definite answer because they have not examined you personally and looked at your diagnostic test results.

The decision to either watch and wait or undergo treatment would have to be made by you and your surgeon and would depend on several factors such as severity of your symptoms, as well as the pros and cons of each treatment alternative. I would advise you to sit down and talk with your surgeon about these treatment options for you to make an intelligent and informed decision about your condition.
