slapping himself

My son is 3 years old.  He has a hard time expressing his anger.  He will slam doors and then my husband and I will tell him he has to sit in time out for that.  We let him know that he has to communicate and say "I'm Upset".  However it started to get disturbing when we put him on time out in the chair and he started slapping himself across the face.  He has done it twice and my husband and I tell him to stop.  Is there something serious going on here?
