burning foot pain

I am a 56 y/o male with a 1 year history of burning pain, paresthesias on the plantar aspect of both feet.  the pain goes back and forth between the two feet.  I have been evaluated for radicular pain including mri, and ct.  There is no evidence of spinal stenosis or discogenic problems.  I am not diabetic.  Insulin resistance test was 11.5. glycosolated hgb is normal.  Thyroid profile is normal. mma is normal.  I had a calf biopsy from Therapath labs in NYC.  The DX is small fiber neuropathy.  my 5mm punch biopsy shows a result value of 4.82,  I understand that anything above 5.0 is considered normal.  I have tried almost everything including pregabalin, cymbalta, elavil, tramadol.
I am considering trying calcium or sodium channel blockers.  The interesting thing is that when I stand on a cold surface or take my shoes off and elevate my feet, the pain completely goes away.  I have no pain at night while sleeping.  I do have sleep apnea and use c-pap.  Since there is sometimes redness and heat on the balls of my feet and in the interdigital spaces, I am wondering about erythromealgia.  I would appreciate any help or recommendations you can give me.
