Food allergy?

I have a 4yo boy who was breast fed for 1st year. He was put on soy formula when weaned. He was diagnosed with vesicouretal reflux which we did antibiotic therapy for 1.5 years. No other health issues. He has recently developed a scalp rash, dark circles under his eyes, loose stools and behavior changes (not for the better). Could this be an allergy of some sort?
