Semen leaking

Is it unusual for a man to leak semen in his underwear if there has been no sexual arousal or recent sexual activities?  Does semen only leak if there has been some sort of arousal?

Semen leaking can be due to arousal.

Other possible causes can include infection, or anatomical disease.  Obtaining a urethral swab and urine culture can evaluate for infection as well as STDs.

A cystoscopy can be to done to exclude a urethral polyp or other forms of lower GU disease that can lead to the symptoms.

These options can be discussed with your personal physician.

Followup with your personal physician is essential.
