Intense Cramping

I have been to a heart doctor about this with no luck, I am hoping someone here can help me... here are my symptoms...

*cramps: chest around heart area, all through stomach and abdomen, and upper back around heart area.
*it has kind of a dull ache all the time and then occasionally tightens up. my chest feels like someone is squeezing my heart.
*it usually makes me nauseated, too.
*sometimes it gets aggravated if I'm driving in a car for a long period of time.
*lying on my stomach sometimes helps the chest cramps, but it takes quite awhile and usually doesn't help much.
*pressing on my back or rubbing it sometimes helps a little.
*nothing helps my stomach.
*it usually goes away after a few hours.
*happens a few times a week at the most and sometimes it happens every night.
*it usually happens at night
*it gets worse if I'm cold.
*it usually goes away overnight, if i can sleep. (but not always)

Did the doctor suggest a blood panel,  stress test or other cardiac diagnostic procedure?  How old are you and do you have genetic or lifestyle risk factors such as tobacco, drug and/or alcohol abuse, unusual weight loss or weight gain, or unusual stress?  Do you get these pains when you are exercising?  

I have a history of cardiac and GERD problems.  I get symptom like you describe from GERD attacks.  Cardiac problems that lead to Angina or in a couple of cases,   heart attacks, occur when I'm emotionally upset or when exercising.

I did take a blood panel...all clear
I have not had a stress test
I have had an ekg and a couple of other heart tests...

No known genetic problems and absolutely zero lifestyle risks...and I don't exercise as often as I should.  I really appreciate your response and any future help I can get!!!  :)
