Cold / Painful Arm

A few years ago I suffered a water skiing accident and hurt my left arm / shoulder. I tried physical therapy this spring and it did help. Now that the weather has turned cold so has my arm. There is a constant pain and it gets really really cold, esp. at night. I cannot sleep on this arm and the pain keeps me up at night. I am a 44 yr old male and I hate complaining or going to doctors. I think there may be a circulation issue. Should I be concerned or seek a second opinion ( my doctor told me to just do the PT ). Thank You, NH
