Child with persistent Lymphadenopathy

Thankyou in advance for any advice.
I'm writing regarding my son who is almost 4 years old.  
For one year he has had a very enlarged lymph node in the right side of his neck. It has never gone down in size, but has gradually gotten larger. It is not painful, and is quite hard.
I am wondering WHAT could cause a lymph node to be this enlarged for so long, and if the fact that it has been so enlarged for almost a year can rule out anything serious. I have attached a picture.
