Post-partum MI

by Jule46720, Dec 23, 2007 09:46AM
I a 38 yrs old police officer and had a heart attack 8 days post-partum. Due to my age, I was followed closely by a specialist from the 12th week to delivery whereby I had a c-section after 26 hrs of labour.  I have no "known" risk factors as I am in excellent shape, gained on 17.5 lbs while pregnant, low cholesterol, low blood pressure and no family history!  My medical team have stated that I will remain on Coumadin for life - "just to be safe"! Currently, I take Lipitor, ASA & Coumadin daily.

My questions are:
1.  How common is it for women to have a post-partum MI which orgininates  in the heart?
2.  What is the normal course of treatment for this in a woman with no risk factors?
3.  If I NEVER get pregnant again, what is the probability of my having another clot?

Thank you.

I wouldn't think it is especially common.  Did you receive an echo, where they do a doppler of the heart, or a cardiac catherization to see if you have atherosclerosis (clogged arteries)?  There is a condition called peripartum cardiomyopathy where clots can form in the heart due to the heart being weakened during pregnancy.  That happens without risk factors, approximately 1 in 1,300 to 4,000 births.

Please make sure you know for certain the cause of the MI.  It could be the normal changes that cause you to clot postpartum caused this, sort of an overkill by the body.  Some women experience clots after delivery, yours just happened to be in a very bad spot (some are in the legs, occasionally the lung).  It may have been a fluke thing and you'll never have this again, but make sure they follow up on this to ensure you do not have a clotting problem.

As for the permanent coumadin, yes, some doctors prefer the safe route.  Sometimes they will wean people off of it, it depends on the situation.

If you never get pregnant again, your chances of a clot would be less, but by how much I would have no idea.

Congratulations on your new baby!
