19 month old headbutting

My 19 month old has a problem headbutting.  He does not do it softly and most of the time it's on a hard surface or a person.  He does it sometimes for no reason that we can find.  He'll sit and look at you then bang his head on the floor, wall, door, just whatever is closest to him.  He also does it out of anger at times.  Any suggestions that you can give to help?  I have tried timeout, telling him no and just ignoring it but nothing is working.   I'm afraid he's going to seriously injure himself.
