anal fistula

My 26 year old son has developed a high complex anal fistula. At recent surgery it was found to be impossible to lay the fistula open and he has had a loose seton inserted which has been in place for a month. Is it possible to leave the loose seton in place indefinately or will other treatment need to take place?

Hi Chubishka,
The placement of seton is only a temporary measure to aid healing of the fistula.
It is generally removed later .

My son was also operated for anal fistula five weeks ago. The wound is taking long to heal.Besides a few drops of blood when cleaning that area there are no other symptoms. Could you please give me some advise.

Wound healing generally occurs within 6 weeks. Proper wound care and regular bowel movements ( with high fiber diet) will expedite the process of healing.

i'm dealing with anal fistula in a year my doctor tried the AFP but it didn't work now he said he will try the seton tightning procedure.can you explain to me what is that mean ?and hw successful it is? To help cure the fistula. is there any possibility of loosing the control of bowl movement?........

me too i have an anal fistula i have a seton inserted for about a year now my doc tried the anal fistula plug but it didn't work.
the loose seton is just temporary. your son has to see a colon and rectal specialist.

A seton is a rubber band like material that is threaded through a fistula so that each end is tied loosely on the outside skin. It is a temporary measure to drain the fistula before surgery can be done.

Fistulotomy is the surgery done for fistula which consists of unroofing of the fistulous tract. But there are chances that the muscle may spring back closing the tract (roof) and forming a fistula again

In seton tightening procedure, after the seton is placed, the skin is incised over the fistulous tract and the steon tighetened gradually over several weeks so that it gradually cuts through the muscle. Since it is a slow process, scar tissue forms preventing the muscle to spring back.

To make it simple -
Seton tightening procedure = a slower process of fistulotomy with a seton.

The type of surgery done for anal fistula depends on the type of fistula. Even after careful selection, none of the surgeries is 100% successful. A 10% recurrence rate is always there.

Some of the precautions to be taken-
- Keep the area washed and clean with plain tap water
- After passing stool, wash properly with a stream of water
- Drink plenty of fluids
- Take high fiber diet.

thank you for your very quick answer and thank you for the time you give to helpless people.
after talking to my doc i talked to him about having a fistulotomy he mentioned that he want to leave it as the last choice and he said in women is different then in man cause ther is a big chance of  cutting the muscles but he didn't explain. does he mean ther is a big chance of incontinence !!!!! wich is scaring me to death.
with the seton tightning is the incontinence still present?  and does the incontinence means a total uncontrol of bowel movement for the rest of my life.
your answer is very apreciated.
