Denosumab is a promising new drug which at the present time is still undergoing testing regarding the best way to maximize delivery. In general it seems to have a profile that is pretty similar to zoledronic acid in terms of preventing fractures, but may be able to work in more patients (there is a subset of patients in which drugs like zoledronic acid do not seem to work well). If you are participating in a trial I would recommend you take the denosumab because dosing would certainly be optimized in a trial.
Outside of a trial setting, it may be safer to go with zoledronic acid as its use is better established.
