First there are alot of aspects of a semen analysis besides count, infact morphology ( the way the sperm look is actually a more significant factor). I would first see if there is a urologist in your area that is trained in fertility, an make an appointment. There are occaionally some anatomical factors affecting sperm. There are also some genetic factors that a REI and a Fertility trained urologist might be able to test you for.
As a very vague general rule I think of this, if count is the only factor:
20 million/cc = normal
10 to 20 million/cc = Intrauterine insemination
5 to 10 million/cc= in viro fertilization
1 to 5 million/cc= in vitro fertilization with intracytoplamic sperm
The treatment for male factor infertility has been treated with technolgy: IUI, IVF, IVF/ICSI.
Hope this helps, JM
