Ovary removed 6 wks ago, what next?

I was diagnosed with PCOS in 1994.  I found out I had PCOS at the same time I found out I was pregnant with my daughter, who is now 12.  I had surgery 2 months after my daughter's birth to have tumors removed from my ovaries.  They had to remove more than half of my right ovary then.  I'm 34 years old now, and just this year I found out that the tumors were back, dermoid tumor on my right ovary.  My GYNO removed my whole right ovary this time.  So now all I have is a small amount of ovary tissue on my right side.  I also had LOTS of scar tissue removed due to the previous surgery.  It's been 6 weeks since the surgery, and I had a period, and i've been taking fertility tests to see if I have a LH surge, but nothing yet.  I have regular periods, but I am overweight.  I was on metformin prior to the surgery and lost 20 lbs.  Should I get back on the metformin?  What's my next step in trying to get pregnant?  How many months should I try before seeing my doctor again?

I am a bit confused. You have written that you have had your entire right ovary removed, and then state that all you have is a small amount of ovary tissue on your right side? Do you mean a small amount of ovary tissue on your left side?

In my opinion, how to proceed depends on how long you have been trying to have a baby. If> one year (or greater than 6 months if you are older than age 35), then you should have an infertility evaluation to include a semenanalysis, and some testing (either a day 21 progesterone, temperature charting, or urine ovulation prediction) to determine if you are ovulating. You should have your operating note reviewed from your recent surgery to determine if you have any scar tissue affecting your fallopian tubes and/or an XRay called a hysterosalpingogram to see if your fallopian tubes are open.

If you have been trying for less than one year, you probably should just wait and see what happens with your periods. If they are regular and ovulation prediction confirms ovulation, it would be reasonable to just keep trying until a year of trying before seeing an infertility specialist. If they are irregular, then you would need to take steps to start ovulating, which could include taking metformin. I would only use the metformin if you find you need medication to help you ovulate, possible with clomid as well.
