Lasttime I had a ear infection and was sent to an ENT becuase the ear canal was completed close he explained to me that I had a big hole in my nose.  This was a year ago.  Well now I am having trouble sleeping(total Insomeia) , dizziness and head aches.  I feel like I'm always out of breath and fatigued.  Does any of this sound like anything.  The dizziness has occurred twice in the last 2 months to the point I could not drive or work or keep my eyes open for 3 days. I slept all day because of the medicine the doctor is treating the dizziness for.

From your description of "a big hole in my nose", it sounds like you have a perforation of the nasal septum.  This is a serious problem that can result in turbulent airflow through the nose, excessive drying and crusting and recurrent infection.

It is very unlikely that the perforation is the cause of your insomnia, dizziness, headaches, shortness of breath and fatigue, with one exception.  If the perforation has resulted in disturbed/non-restful sleep, the sleep deprivation could cause at least some of the symptoms you describe.

You should ask the ENT or your PCP if this is a potential problem and if the hole in your nose, presumably of the nasal septum, the tissue that divides the inside of your nose into 2 compartments, should be repaired with a "tissue flap."
