pulmonary hypertension ecocardiogram question

I had an echocardiogram, which showed RVSP of 40. It also showed that I have a mild pulmonic regurgitation and a possibilty of small patent arteriosis.
1.Could echocardiogram show wrong numbers, what influences that?
2. Is there any other test that is not as invasive as Catheterization that could provide more accurate measurement of whether I have PH or not?
3. Can patent arteriosis cause PH even if it is small, what test could help detect that?
4. What is the cause of pulmonic regurgitation?
Please help !
