Obesity/PCOS/Anovulation/Met

Though I have never officially been diagnosed with PCOS, my dr. feels very strongly that I do have it.  Allow me to give a brief synopsis of my situation/health.  My husband and I have been TTC for a year now, and I have been on Metformin for 5 months.  I was on birth control pills for 6 years and stopped one year ago.  First three menstrual cycles were normal (heavy) 28 days for me.  Then I went 5 months with hardly anything.  Some months I wouldn't have a period at all, or a day of bleeding, then nothing.  It has made it difficult to predict ovulation to say the least.  
Recently, I have had two cycles return after being on Metformin for 3 months.  I'm on 1000mg per day.  My dr. advised me to lose weight along with the Metformin, in order to induce ovulation.  I am (shamefully) 375 lbs. ---obese!  So far I've tolerated Metformin quite well, in fact, I haven't had any of the nasty side effects that I've heard other people endure.  I wonder though if increasing my dosage wouldn't cause those side effects to appear?  
Anyhow, dr. wanted me to try and lose weight and stay on 1000mg of Metformin, until he sees me again in March.  He says we'll consider Clomid as an option at that point, but he wants to take this route first.  I'm 34 years old, and will turn 35 in April.  I'm terribly afraid that my "window of opportunity" will close on me to get pregnant and have children-- being obese and older.  
Has anyone had any success with a similar story?  
At this point it would take me a year or better to get to a healthy body weight for conception/pregnancy, and in the meantime, I'm getting older and the biological clock ticks away.  
Need some help and hope here folks.  I'd appreciate your comments/suggestions.

Hi Melisa,

I think your doctor's plan sounds rational.  You should work as hard as possible in the meantime at losing weight.  You may not get down to  'target' but any weight loss will help both you and your chances of having a healthy safe pregnancy.  Between now and March you could lose 10-15lbs.  

As you lose weight, your chances of conceiving naturally will increase. You still have 5 - 10 years of childbearing opportunity ahead of you, presumably you don't want more than 2 - 3 children, so there is still time... so focus on getting and staying healthy.  If you do get pregnant and have the baby then managing your weight so you can look after and enjoy your child is also critical.

If you had no side effects from metformin at 1000 mg, likely no problem even at a higher dose.  But given that your periods have returned to regular on 1000 mg I would reckon that the dose you are on now is suitable.

Have you tried something like weight watcher's?  There are also on line diet supports groups for free.  

All is just my personal opinion.  Best wishes
