Unsuccessful Ablation

by chel34, Feb 05, 2008 09:20PM
My ablation (1/23/08) to hopefully rid me of suspected AVNRT was unsuccessful due to the problem current originating within 1mm (according to my EP cardiologist doing the procedure) of my AV node. Apparently a risk of the procedure by only 2-10% of cases, depending on which article you read.  The procedure itself was not that unpleasant for those of you considering it.  I chose ahead of time not to proceed if found to be risky because damaging the AV node would mean a pacemaker for me.  I am 34 and mother of two young boys and a pediatric OT who NEEDS to stay active!

Now I got the results from a recent ECG (11/27/07) and ECHO (12/18/07)...
ECG -
borderline R wave progression, anterior leads R<.15mV V3-V6

and more concerning ECHO -
moderate left ventricular systolic dysfunction with ejection fraction 40-45%
dilated and mildly hypokinetic right ventricle
mild bileaflet mitral valve prolapse with trace regurgitation

Now the two big questions since you have the history... since the ablation was unsuccessful I've started a new drug, Flecainide.  The first few days were icky, I was on too high a dose and felt tired, dizzy, and not myself.  A decrease in the dose has proved effective in controlling my palpitations and racing beats, but here is my concern.  Flecainide works by blocking sodium channels thereby decreasing overall ventricular contractility.  Given my decreased EF already, would Flecainide make it worse?  Does anyone think my ECHO results speak of something more than a rhythm problem, i.e. sclerosis, etc?  Any input is appreciated.  Thanks!
