Abdominal/Pelvic Pain

Hi,
I'm a 32 yo female who has been experiencing severe pelvic pain everytime I get my period.  On rare occassions, the pain is so severe that I vomit and black out.  About 20 mins after I pass out I feel a whole lot better.  My hearing goes and my vision is gone ie, I see black.  I have had ultrasounds and tests and nothing seems to evolve.  I have been diagnosed with PCOS but apparently this is not associated.  I have just started a treatment of Diabex which is Metformin as I have been trying to fall pregnant for over 1.5 years and blood tests revealed that my insulin is not working like a type 2 diabetic.  I weight 55 kgs and am not overweight which is unusual for this condition.

Yesterday, I was at the gym and the pain was brought on by exercise, and once again, I passed out.  This was the first time I experienced the pain without having my period.

What could this be and is it related to PCOS?
