Hi,

After whey too much alcohol on a night out in Madrid last week, I ended up in a nightclub with rooms downstairs for extra activities!  I was given un-protected oral sex, and gave oral sex to two women.  There was sex, but this was protected and there was no problem with the protection.  During all of this, I also smoked (which I never do)....

The next day I had a soar throat, which I put down to the smoking, but this has continued (6th day now).  The following day I had what appeared to be a bad mouth ulca (white and about 1.5cm wide) - this cleared up in a couple of days after using Bonjela.  My lips the next day were also very dry and cracked within a day as if I had been out in the cold (also ok now due to lip balm).  Now they seem to be dry most of the time, hence the lip balm.  In addition to this I have started to get some small spots around my mouth and nose, today a couple on my neck also.  They are very small, with a white cap (pus I guess) - the kind perhaps I would get after eating the wrong food too much!

My question here is.. does this appear to be anything I should be visiting the clinic for now (which I probably will do anyway to ease the worry), or is this all being caused by worry of being really stupid?

Symptoms immediately after sex (i.e. within 18 hours) are almost never due to STD.  Too short an incubation time.  Cracked lips -likewise unlikely to be an STD.  Finally, a rash on your mouth, nose and now neck - unless you really managed to cover yourself with secretions this likewise seems unlikely to be related to your exposure. To be honest, I see little to worry about.  Having said that, I would suggest, as you've already concluded, that it is worth your while to seek the advice of a clinician who could take a look at the rash.

By the way -no way you got HIV from this exposure- that just doesn't happen enough to even be mentionable.  As for other STDs, there are possibilities which sometimes come from receipt of oral sex but they are, to be honest, uncommon.  The most common STDs transmitted through receipt of oral sex are NGU, gonorrhea, or herpes due to HSV-1, if you have never had cold sores (the immunity from cold sores is protective) and the person giving oral has a cold sore present at the time of sex. Your symptoms do not match up well with any of these.  Chlamydia and HSV-2 are effectively never tranmitted in this way.  Syphilis could be but it is very, very rare.

As far as exposure of your throat during the episode, gonorrhea is the only real major concern.  This is usually asymptomatic and can be diagnosed by culture.
