What records are created by doctors or staff that establish history which can be updated for the patient for this event and possibly follow the patient or be consolidated in the patient's other records?

It varies with the protocl of the institution. Many doctors and hospitals have done away entirely with written records and rely upon computer patient records. The old way produced illegible notes, but the new way often causes much important information to be omitted. You have to request all your records periodically. Some hospitals and physicians hand everything over. Other facilities make it extremely difficult.
