The risk of STDs other than HIV from the exposure you describe was very low.  STDs are not known to be transmitted at all by fingering, including finger-anal contact, even with genital secretions as lubricant.  Likewise, the risk was low from the unsuccessful attempt at penile-anal insertion.  Nobody can say the risks were zero, but they are theoretical without any cases known to have been acquired by such exposure.

As you apparently know, unexplained skin rash is always a reason to test for syphilis. However, your rash came on too soon (the generalized skin rash of syphilis occurs in the secondary stage, which doesn't begin until at least 2-3 months after infection); and in any case, the negative syphilis blood test at 2 months is solid proof against syphilis.

As far as abdominal pain goes, certain gastrointestinal infections are common from male-male sex; common causes include campylobacter, shigella, and salmonella.  However all those generally include diarrhea and fever, not abdominal pain alone.  No other STD would cause such a symptom.  Innumerable non-STD things could cause your other symptoms, and your test results prove for sure you didn't catch HIV.

If your rash or other symptoms persist (or recur), continue to follow up with your health care provider and follow his or her advice about diagnosis and treatment.  But you can be sure you have nothing you caught during that sexual encounter.

My last thought is that you need to get safe sex principles into your head for any and all future sex with men.  No male should ever have sex with a new male partner without having condoms handy and using them for anal sex (there is no excuse for allowing a partner of unknown HIV status from attempting to penetrate your rectum).  Also, always ask about and share HIV status.  Follow both those rules, and you probably will go a lifetime of safe sex with other men, without HIV.  Fail to follow them, and there is a good bet that HIV is in your future.  It's that simple.

Best wishes--  HHH, MD

Sorry, I missed the condom part.  Don't take my little "lecture" personally.  I make the same point whenever I can, to educate all men who might read the thread.

Aside from syphilis, no STD causes a skin rash outside the genital area.  That's as far as I can go.  Whatever it is is not an STD and not related to the sexual exposure you described.
