4y.o. death, dismemberment play

I nanny a four year old who put on a play for me and her parents about death.  She positioned 10 dolls on the floor took one of the dolls (quietly talked to her) then proceeded to "kill" the other nine dolls.  Then through this one doll cut off the other 9 dolls heads, hands, and feet.  Then "took" their blood and skin.  The end of the play "everyone lived".  
This child does not have a TV in the home, and attends all day kindergarden (strictly supervised), and has three older sisters (who never shown this behavior).  
My question is should I be concerned for this child?
The Mother told the child she should not be putting on plays about death.  The Father told the child that "nobody cuts off your head, hands and feet after you die, you just get put in a coffin".

That's odd, I would expect that maybe she would see something like that on tv but if she doesn't have a tv... I wonder if she saw a video game or something at a friends house/friend's big brother, etc.   She's still young but that's quite graphic for a such a young child to make up.  

I would be concerned.  Since you nanny her, can you ask her where she saw that?  Try to talk to her and if she says anything, let her mother know.  Hopefully, her mother will take the rights steps if it comes down to it.
