White nonpainful spots on the face especially on the lip areas may be clogged sweat glands, blocked follicles and enlarged glands which lines the lips. Other differentials may be pityriasis alba, vitiligo or a fungal infection. If this has been present for almost a year, then fungal infections and vitiligo may be considered. Fordyce spots may also be likely. "Fordyce's spots, or Fordyce granules are small, painless, raised, pale or white spots or bumps 1 to 3 mm in diameter that may appear on the shaft of the penis or on the labia, as well as the inner surface and vermilion border of the lips of the face."

Continue to observe these white spots. They do not need immediate intervention unless there are pain, redness, discharge and fever.
