Rapid Follicle growth with Femara yet no LH sugge

I am on my second cycle of Femara and noticed one common thing in both of my cycles. I see that my follicles grow very rapidly with Femara, unlike my 6 cycles on clomid where the follis will struggle to reach 18mm on CD16-17. My lead follicle was 32 mm on CD14 in the last cycle of Femara, so my RE asked me to come in early in the next cycle. So in my second cycle I went in on CD12 and found my follicle was 28 mm on CD12, that's when they gave me trigger shot this time.
1. My question is that am I not supposed to get a LH surge naturally when the follicle grows like 18-22mm and then I should be ovulating. Why at both times I did not get a surge at that size? Doesn't the dominant follicle produce the hormone needed to trigger ovulation naturally when it reached a good size?
2. I know they say that the follicle is optimal for ovulation between 18mm-22mm. However, what is that maximum good size follicle for ovulation and conception? Did I much surpass the maximum good size both of these times?
