The term, "old granulomatous disease" is based on x-ray findings that show scarring, characteristic of past infection with any of a number of fungi or tuberculosis. The implication is that whatever infection caused this scarring, is currently inactive and that no treatment or further testing is required. A full interpretation would require knowledge of the reasons why the CT scan was performed and you should discuss that with the ordering doctor.
