In the middle of fog

Please can someone answer these questions?
1. An EMG tests for neuropathy and myopathy - is it possible to have a clean nerve conduction study but a problem with the needle EMG?
2.  Can you have a myopathy without and nerve damage?
3.  With ALS would you have both a neuropathy and myopathy?

Ah I forgot to add,
4.  Would a TSH of 8 and subclinical hypothyroid be enough to cause muscle weakness?

Does definately not having a neuropathy rule out any lower motor neurone disease?
