lacunar infarction with hemiparesis in a handicap person

my father age 46 diagnosed as  lacunar infarction ,periventricular.ct done. had a history of TAO. ABOVE KNNEE AMBUTATION LEFT LEG  before 2 years. nw suddenly become hemiparesis .,diabetic . bt he ws non diabetic ,and hypertensivenow on aspirin, manitol,glyserine. having bowel and bladder.he is in aregion of india wer gud treatment is not avaluable. im a staff nurse in ALL INDIA INSTITUE OF MEDICAAL SCIENCES, NEW DELHI INDIA . so i would like to what will be the prognosis ,  wt should we do?
