testicle tender/ pelvic shooting pain

Dear Doctor


i started to get a throbbing  testes and tingling in them painfull to sit/run.lasts 4 weeks
Week 7 went back to std clinic for uretha swab /urine test (all clear)
Week 8 Testicles are very tender on the underside to touch and I  dont want to excersise due to soarness.
week 9 & 11 i have been to see a urologist twice , she is unsure! she says the testes feel normal, are not red /hot and no sign of infection. She gave them an ultrasound no sign of cancer/ infection
She tested my prostate and i felt and I felt no pain in testes.(so apparently that isnt the cause)

Ultrasound found some fluid on my lower stomach ( although  the size was measured and i was told not enough fluid to cause pain) she recommended to see another urologist as she was unsure.

Sympoms are tender testes all the time soar if squashed ie driving/ tight trousers , shooting pains lower stomach / pelvic area ,sperm seems to be getting much thicker and less ejactultion .,no fever, no pain urinating, i have had 4 small discharges, of sperm like substance.
Biggest concern is the tender testes and sharp shooting pain in pelvic area .

Also a small white dot 3mm (not raised) has appeared on my penis head
along with a small black 3-4 mm dot  both of which no one thinks much off on the shaft (not raised)
testes feel like being sqeezed feeling when the underside is touched.
short sharp pain in the lower pelvic area 10 times a day

Question is do you know whats wrong? what else can i do ?
Is it possible i have an std that hasnt been detected from swab / urine If they were not looking for it, I am of the opinion the swab would show any bacteria infection  causing my pain/discomfort.
Could this be a virus or prostate infection ?

I was originally concerned of an std due to a handjob, for info,  thatsbeen ruled out apparently but something is clearly wrong
hope you can help

Sorry for the delay in getting back to you.
I do not have any explanation for your pains on the basis of your description. You have seen a urologist, which was the sensible thing to do, but she cannot give you an answer. She has performed the reasonable tests and they are all normal. In circumstances such as this, I seriously doubt that there is anything seriously wrong. In situations such as yours, I start looking for emotional causes. Some of us express our anxieties in physical ways. I, for example, get ulcers when I am stressed. Treating the symptoms and not dealing with the cause leads to short term improvement at times, but as the real reason is not addressed, the problem is likely to return.
I suggest that you request an emotional evaluation and see what that expert thinks.

For ref. in the last 2 weeks my stomach has felt very bloated and feels very uncomfortable. along with shooting pains right through my penis, and discomfort on my hips
As for the emotional part i can assure you i'm mentally strong.

Can you really  not think of anything else that it could be other than pressuming mental issues?

"Mentally strong" and expressing your anxiety in the form of physical symptoms have nothing to do with each other! You are not physically weak because you develop an infection, an ulcer, or a cancer. Nor do you have a severe mental illness if your symptoms are of emotional origin. I will tell you that the longer your symptoms go unresolved, if they are emotionally  based, the harder it will be to resolve them.
No, I cannot think of a physical cause that would tie together everything that you are experiencing.

Dear sir as mentioned i have been back for more tests at 2 different places
At the std clinic the doctor felt my testes the exact point of pain on the left side and slightly on the right,
He said it was Epididymitis
I believe him and he gave me Ciproflaxin for 23 days twice a day 500 mg
He couldnt believe no one else had spotted it!
Considering i have had azitromax3 day course (twice) and apocilin for 10 days do you think this will help?

The next day i had a urologist booked and i thought i should go as i had to pay anyway, the urologist snubbed this diagnosis and said i had maybe a hernia and to go see a stomach specialist
