lower back/kidney pain

My uncle is having some pretty bad lower back pain and i was wondering if you all could give any insight. It hurts when he stands but it is relieved some when he sits down. He does take blood pressure medicine and drinks a lot of diet cokes. He has been adding water to his diet though. Any clue what this could be? Just a kidney/bladder infection? Unfort he doesn't like going to the doctor, so does anyone have any info on the possile reason for his pain, as well as what he might do to make it better?
