Enlarged Prostate, Flowmax and priapism.

by brewer3620, Mar 30, 2008 07:04PM
About two weeks ago I started having frequency in urinating, back pain, and pain in my testicals and pain in the area between my rectum and my testicles.  I had no burning or discharge.  I saw my GP and had urine test which showed no blood, bacteria etc.  I had a blood test done and white blood cell count was normal.  I then had digital rectal exam and it was normal..  The Dr. stated that this would have been very painful if the prostate had been infected.  The Dr. stated prostate felt fine with no nodules.  The prostate was however, enlarged.  The Dr. ruled out prostatis and diagnosed as enlarged prostate. The PSA test has not returned yet.  I 've been on doxycycline 100mg twice a day as well as pain meds.

Question number one:  With this information is this diagnosis accurate?

The Dr. then prescribed Flowmax (oral) to relax the muscles around the bladder etc.  A side effect of the medicine (1 in 50,000) states that priapism is possible.

Question number two:  I have not had sexual intercourse or ejaculated since being prescribed this med.  I have had several erections that went away with no problem.  I was curiuous as to if this would rule out the possibility of priapism or if the priapism the warning is referring to is being unable to get rid of an erection after ejaculation?  I am confused in this area.  Your advice is greatly appreciated.  Thanks in advance.

I agree with your physician, this is not prostatitis. The tenderness that some men experience with a digital rectal exam has more of an association with age than much else. Younger men seem to be more tender than older ones, in general.
The Flomax is a reasonable drug to deal with your symptoms, which some might diagnose as "prostatotosis," but I like the term, "perineal muscle myalgia.' The medication may only need to be  taken for a few weeks to months.
Priapism is a painful, sustained erection in the absence of sexual stimulation (though it may have started  with sex). I have never seen this with Flomax, myself. It's occurence is quite infrequent. I would not let this possibility influence you decision to take the drug. Note, a percentage of men fail to ejaculate when taking  Flowmax. Uroxatral, one of the other drugs in the same class, does not have this untoward effect (which is reversible). Failure to ejaculate does not harm you.
