Lynph node the Problem ??

Recap: Diagnosed with Pneumonia per X-ray late Dec 07 (sharp right chest pain). Levaquin worked. Re-occurred 2 months later (sharp right chest pain) X-ray & CT. Levaquin worked again. Coughing 40-50 times a day.
Blood,TB, sptutum, urine all negative. CT scan report states :
Atelectasis in lateral middle lobe, may be partial atelectasis in medial segment of middle lobe. "A soft tissue nodule 16 x 11 mm (most likely a hiliar lymph node) is present. Not clear if this node is causing compression on a portion of middle lobe bronchus. The fact that no change from prior X-rays may indicate non-obstructive middle lobe collapse".
1. Is the presence of the hiliar lymph node unusual – what is the cause ?
2. Could the node be the source of the problem ? If so, can a bronoscopy resolve this lymph node problem ?
3. What causes non-obstructive middle lobe collapse and how is it treated.
What is your advice our advice ?
