Gonorrhea?Anal Fissure?

Here is the sequence of events which have led me here.
Jan 6th - cheated
   -recieved and gave oral (unprotected)
   -recieved oralingus (unprotected)
   -recieved anal sex with a condom
Jan 7th
   -masturbated with object to stimulate prostate (object is organic and porous material)
Jan 9th
   -began having symptoms
       -urgency to void bladder
       -general pain and discomfort in entire pelvic region
       -went to clinic for std testing
       -swab test for chlamydia/gonorrhea, blood tests for HIV/syphilis
       -prescribed antibiotics to begin while awaiting test results
Jan 15th
   - all tests negative
   - had already taken the "all at once" antibiotic on the 10th(four pills)
   -had taken five days of 10 day antibiotic course
   - stopped all treatment at this time
Symptoms went away after about three weeks on their own.

Feb 23rd
   -Again used same object in anal masturbation
Mar 1
   -Symptoms return
      -urgency to void bladder
      -pain in all of pelvic area
      -stopped all sexual activity
Mar 4
   -return to doctor
   -Dr. suggests possibility of herpes, prostatitis, epididimitis
   -Dr. is not informed of anal masturbation, only cheating history
   -Dr. prescribes acyclovir as a guess
Mar 11
   -Acyclovir ineffective, no sores or blisters manifest
   -Dr. prescribes 30 days of Cipro
   -Dr. does rectal exam, urine, and blood HSV tests(neg)
With Cipro
   -symptoms persist
April 11  
   -receive antibiotic shot and one dose powdered antibiotic (in case original swab test was false)
my symptoms:
    urgency to pee, stinging pain in tip of penis, pain in urethra and base of penis, tingling in scrotum, testicular pain (later in day),perineum pain,skin irritation, pain in rectum,mucus with formed stools, pain in groin muscles and thighs, lower back pain (both sides),
I HAVE NOT HAD ANY OF THESE SYMPTOMS:
discharge
blood in urine, semen, or stool
fever
burning during urination
pain DURING movement

I would like to help, but really cannot.  No distant online source, no matter how expert, can come up with answers that are not clear after multiple direct examinations and tests.  Anal fissures have lots of causes, and STDs are not high on the list.  All in all, it sounds to me like the problem is primarily traumatic. Further, analingus probably rarely if ever transmits STD to the anal area, and your only other anal exposure was condom-protected.  So STD really wasn't a serious consideration at the outset. The standard tests for gonorrhea and other STDs are highly reliable.  Those negative results plus lack of response to the treatments aimed at various STDs pretty much prove that no STD is responsible for your problem.

33yo, male, jan was only second homosexual exp. 1st was over 10 yrs. ago.
could gonorrhea just dissapear like that, weeks after discont. treatment, and then return?
what non-std's could cause all these symptoms?
what questions should i ask the urologist when i see him?
I've read that there are "gay" stains of gono. resistant to normal treatment.
would gono. infection have shown up in CBC or urinalysis?

Statistically speaking, gonorrhea strains that infect gay men are more likely to be antibiotic resistant than in straight men and women.  (The rectal environment selects for antibiotic resistance.)  But even the most resistant strains respond to the standard treatments.  And the standard diagnostic tests work equally well in gay men and straights.  Gonorrhea is not a plausible explanation for your problem.

thanks for the help! trauma? makes sense to me! What can I do treatment-wise? I have a urologist appointment next week. Should I save the $300, and try home treatments? Or, are there serious possibilities that he should rule out?

You could see a urologist if you wish.  But first read about the male chronic pelvic pain syndrome. Google that term to see lots of stuff.  Start your reading with the excellent wikipedia article.
