aching, twitching, twirling oh my!

I have experienced multiple increasingly neurological sx for a couple years, but in the last couple months significantly increased in frequency and intensity.  I have had off and on paresthesias for a long time, because I have taken several neurological drugs for bipolar disorder which have that as a side effect.  For the last couple years, I have only been taking lithium and lamictal.  I can only sit for about 5 minutes before my legs go to sleep. The tendons in my calves (esp the right) ach like tendonitis just randomly.  My right side is significantly worse, and after about 15 minutes I can't bear weight on the right side until after I stand up and massage/shake my leg till the pins and needles go away.  I have no LE reflexes. My hand--usually the right side, twitches and sometimes my index finger and thumb touch--totally involuntary, but I can stop it by using my hand to do something.  Lastly I have migraines that have gotten worse--with significant vertigo and spots.  Do you suppose this is 2 separate issues? migraines plus a nerve thing?  I had an epidural 12 years ago and the dr. hit my sciatic nerve, which caused some tingling and pain, and it was on the r side.  Is that a coincidence?  Why would it suddenly be getting worse?  Could it be something awful like ALS or MS?
