high viral load/low liver numbers?

I'm a 40 year old male who has  had Chronic hep C at least 10-15 years. Don't know the genotype, but I'm assuming it is 2 or 3 since i most likely contracted this through needle use in Asia. I've been clean and sober for 10 years, no longer smoke (3 years) and  75% of my diet is raw vegetables and fruit.
I recently did some tests again because I've been suffering from tremendous brain fog. Here are the results:
AST 32 (lower than last year--48!)
ALT 56 (lower than last year-64!)

BUT my viral load is really high: 22,900,000 copies. (Unfortunately, I had never done this test before, so nothing to compare)
According to the ultrasound, my slpeen is slightly enlarged, and the doc diagnosed moderate fibrosis also using the ultrasound. (also the first time I do this test)
In the country where I am (Taiwan) the insurance wont cover treatment unless ALT is beyond 80. Of course, I'd rather not do the treatment, but I am wondering if my elevated viral load is an indication of major trouble.
Any advice on this one?
