Heavy, irregular periods with spotting between them

Okay, I am 26 with 2 kids, ages 5 and 4. I have had irregular periods since having my second child, but never like this. My cycle has gone from 14 days, then be off for 1 week and start again for 4 days, be off for 2 weeks and started again for a regular 5 day cycle, all while spotting almost everyday between these periods. I have now gone 4 weeks as of tomorrow without a period, but I am still spotting. I have heavy bleeding and bad cramps most of the time while on my period, but usually no or little cramping with the spotting. What could this be? Please Help!
