Doctor, I am a 29 year old office worker who eats very healthily has low consumption of alcohol and exercises - I’m in good health. I have one partner for 5 years and always have protected sex. My problem started 8 weeks ago, I woke up with an irritation inside my penis with no discharge, I had not had sex for at least 2-3 wks. The sexual health clinic tested me, I had low white blood count of 5-10 , I had NSU, they gave me a course Azithromycin. After 4 days I was still experiencing symptoms, urgency to urinate, burning on urination, irritation inside penis so I went back, they checked me again and found I was clear they prescribed me with doxycycline and metronidazle, the doxycycline 1 wks course this felt better until I masterbated the soreness came back. I then took metronidazle 5 tablets this made the situation worse the pain intensified and felt a burning sensation in between my legs (prostate area).  I was prescribed ciprofloxacin for 1 wk no change. Urine has been checked they find blood in the urine but they always find protein with no bacteria present. I then had an x-ray this was clear, I also had ultrasound they found one cyst on my left kidney about 25mm in size. I had my prostate checked tests were clear slight swelling. 6 wks on still getting irritation inside my penis, tip red, feeling unwell, chills and dizziness. I then took erythromycin at first made me feel better but the burning sensation in the prostate came back this died down after the first 3 days, course finished but still have an irritation inside the penis and slight burning after urination these symptoms come and go. I am thinking of getting a two week course of doxycycline to see if this would help but I am losing faith with antibiotics this would be my 6th course. Seeing a urologists soon for further testing.

Do you have any ideas of what this could be or advise?

I agree, antibiotics are not the treatment for this  problem. If they were, you'd most likely be better by now. Your decision to see a urologist is good one. With persistant blood in your urine this physician  may want to look inside your bladder and urethra (cystoscopy). What type of x-ray did you have? If not a CT urogram or IVP,  then one of these will need to be obtained.
