Difficult to pee

Male 53  I have interstitial cystitis and have trouble urinating.  I take Elmiron and IC was tolerable.  It came to a point where I couldn’t pee and got a  foley for 2 weeks before surgery.  I don’t tolerate foleys that well, 2 weeks of utter bedridden hell...My urologist said when he goes in he would determine if I needed a turp or a tuip.  My prostate was 56 grams and he performed a TUIP and everything was ok for a while.  I am also taking Avodart and my uro says my prostates size is ok now.  
Now months after the surgery, the pain and symptoms are just getting worse.  I have never had symptoms anything like this pre surgery.   I wonder if 2 weeks of having a foley has damaged something?  At times it takes up to 2 hours to urinate, not much comes out which means I am probably somewhat dehydrated..  I usually don’t have a desire to urinate, pain tells me I must urinate.  
Lately urinating doesn't relieve the pain as much.   I wake up 2 to 3 times a night because of the pain and have to work at getting to urinate.  I have to be careful if I load up my bladder too much I get bladder spasms.  I am totally exhausted by all this and running out of reserves.  My uro basically said that he can’t understand and put me on 4 weeks or cipro.   Any ideas?
