extremely high hcgs with twin sacs but no heartbeats

Hcgs: 3wks3days 5007; 3wks5days 11700; 5wks3days90085; 5wks5days87400. At 3wks3days no intrauterine pg was seen, just two areas of bleeding. At 5wks3days two gest. sacs were seen, one tucked on top of the other and not completely visible. Yolk sac seen in only the bottom sac. At 6wks3days growth was noted in both sacs, the upper sac still only partially visible and appearing empty and the lower sac with a yolk sac and fetal pole, but no heartbeat. All u/s's were TV. My dr is nearly 100% certain there is nothing in the upper sac(although no abdominal u/s has been done to fully visualize the sac), and that the other baby has stopped developing since there is no heartbeat. My ovulation date is exact, so I know I am not any earlier in this pg. My question is this...is it possible to not see the second baby by TV and not see a heartbeat in the visible embryo with such extremely high hcgs, and is it possible to have a slight decline in hcg at this early stage and still not miscarry. I have had no bleeding or cramping and am still suffering from extreme morning, noon, and night sickness. Thank you in advance for your answer.

you are still very early... you need to just wait it out and see... around 7 weeks everything will be much more clear and you will have h/b

I'm waiting for blood test results now and a scan on tues next week. The dr brought up trophoblastic disease because my hcg's are so high, but I think that is premature because I clearly have two growing sacs and no u/s suspicion of the disease.

they only saw 1 sack for me at 4.2 weeks went back at 7.2 weeks and they saw 2 sacks 2 babies and 2 heartbeats and at 8 weeks saw 3 sacks 3 heartebeats but unfortunatly we lost baby #3 at 9 weeks but they can still miss things on early u/s's

Did they just do t/v u/s with you or did they do an abdominal as well?

Like the other ladies said, it is still so early to tell anything for sure.  I hate the waiting part, but it is an unfortunate reality I guess.  Don't give up hope.  Are you scheduled for another u/s?  Good luck to you.

My last u/s was today (7wks 4days) no heartbeats, but now they found a third baby. Apparently my high hcg's are due to a triplet pg, but now the hcg's have plateaued and with no heartbeats I am told to expect a m/c any day now.

Stay strong and pray. It ain't over til it's over, OK? My doc won't even do the first u/s until 7 weeks, as everything is still developing. A second opinion wouldn't be a bad idea.

Its so early still, anything can happen. And there is so much room for error....not the greatest equipment, skill of the tech.  I have also heard lots of stories here of women not seeing heartbeats when Dr. thought they should.  Hang in there, and we will all be praying for you.

what kind of ultra sound did you have maybe you should ask for a internal ultra sound that shows you a clear picture good luck.

My u/s's have been abdominal and t/v, but since I have a very retroverted uterus they haven't been able to get a good view either way. The dr says the second sac is emtpy (although he saw a shadow of 'something' but couldn't get the angle he needed to see the full sac) and the first sac 'suddenly' had two yolk sacs instead of the one they'd seen all along. They haven't been able to visualize any fetal poles at all yet which is actually the only thing giving me hope, because if they saw the babies without heartbeats, that would be pretty definitive, but since they haven't seen the babies themselves I can hope that it's just the poor angle keeping the babies hidden and they are actually in there with their little hearts beating away in those 'blind spots' that are inevitable with a retroverted uterus.
