Acute Back Pain - Possible Stone

My wife is currently having severe acute back pains that come in regular intervals about every 30 seconds.  It is a pinpointed area on the right side at about the third or fourth rib up.  I thought this might be a kidney stone, but when I had a stone it caused sudden and severe onset of pain that did not pulsate like this and put me on the ground.  My wife says it is severe (10/10) pain for just a few seconds, subsides to almost nothing (2/10) and then returns suddenly about 30 second later.

What in the world could this be?  Should I get her to the ER, see a doctor right away, what?  I am now very concerned.  It's been going on for about an hour now.
